LICENSED PRACTICAL NURSE
Position Summary:
The Licensed Practical Nurse will assess treatment plans and, in collaboration with the Coordinated Care Team (CCT), deliver care for PWH, including the provision of geriatric principles for aging PWH, people receiving pre-exposure prophylaxis (PrEP) therapy, and those receiving treatment for sexually transmitted infections. Additionally, the Licensed Practical Nurse will therapeutic communication work together with patients and their families/support systems, medical providers, case managers, and other care coordination team members (e.g., Community Health Workers, CHWs) to develop and implement individualized care plans to achieve optimal health outcomes for the clients served.
Reporting to the Registered Nurse, the Licensed Practical Nurse will have the following responsibilities:

Experience and Education:
- Valid LPN licensure in DC, Maryland, and/or Virginia required; preference will be given to applicants with licensure in all three jurisdictions but we will work with the incumbent to secure licensure in all three jurisdictions.
- Minimum three (3) years of nursing experience as an LPN; two (2) years of experience in community health nursing with LGBTQ+ adults and/or youth preferred.
- Proficient knowledge of HIV and other sexually transmitted infections (e.g., HIV, Chlamydia, gonorrhea), Hepatitis C, and chronic conditions (e.g., hypertension).
- Current First Aid and CPR certifications.
