The deadline date for making reservations
at the Chicago
Marriott Downtown is June 27, 2005. After that date,
reservations will be accepted on a space and rate availability
basis. If the group rate is not available, the Hotel's rack
rate will apply. The Chicago Marriott Downtown may
assess a fee for early withdrawals.
Room Rates are $155.00 for Single
or Double. Sales and occupancy tax of 14.9% per night will
be applied.

Payments by CHECK must be received by June 15, 2005.
NO CHECKS WILL BE ACCEPTED AFTER JUNE 15, 2005, payments
may be made by money order or credit card ONLY. NBNA
accepts only MasterCard or VISA credit cards. Should
it become necessary for you to cancel your registration,
WRITTEN notification must be received (not postmarked)
by June 15, 2005. NO REFUNDS WILL BE GRANTED AFTER JUNE
30, 2005. Refunds will be made 90 days following the
conclusion of the NBNA Conference. A $50.00 administrative
fee will be deducted from the total amount refunded.
